One proof of residence (Duke Energy, Water, Lease, Mortgage) *Must match the address of the ID. **Water/Duke Energy bill must be from the last 30 days**

EL2 (Physical Form) Upload ONLY page 4 (page 5 if referred to a specialist) ***Make sure page 4 is completely filled out with student information, Doctor signature and information with the medically eligible checkmark, date physical was completed, and student/parent signature on the bottom***

3 online course- Go to NFHSlearn.com (1. Concussion in sports 2. Sudden Cardiac Arrest 3. Heat Illness)

Create a folder on your computer for your documents

Scan each document with Genius Scan

Email the documents from your phone to yourself (skip this step if you are completing the packet from your phone)

Open your email on a computer and save the files to your created folder

Go to https://athleticclearance.com/  (Use Chrome)

Login or create an account: Make sure it is for the 2025 - 2026 school year

Individually go into each tab and upload the appropriate document

Make sure a parent is present. Both you and your parent will have to provide E-signatures

Additional information you will be asked to provide: Health insurance, primary care physician information, emergency contact information, medical conditions.

**Make sure EVERYTHING is filled out. Do not skip a section even if it is repetitive**

Steps:

1.     Go to NFHSlearn.com

2.    CREATE A NEW ACCOUNT or SIGN IN with last years if possible.

3.    Click on Courses (On the left click the student only filter) find the course -- Sudden Cardiac Arrest

a.     Click on the course and select order course.

b.    Then click the course will be completed MYSELF.

c.     Click on CONTINUE SHOPPING next to the checkout button so you can add the other 2 courses.

4.    Complete step 3 again two more times to view the following courses.

Heat Illness Prevention & Concussion for Students.

5.     Once all 3 courses are added – click on the CHECKOUT BUTTON.

a.    Click on the checkbox that you agree to the NFHS policy.

b.     Click CONTINUE.

c.     Click on GO TO COURSES to complete the courses.

6.      You should now see your courses and they will all be listed under your account. Select the first course. You will need to watch the videos and answer the questions. You cannot skip ahead. Once the course is at the end you will be prompted to take an exam and you will receive your certificate if you receive an 80% or higher. Download your certificate or take a screenshot so that these can be uploaded to your ATHLETIC CLEARANCE PORTAL. YOU WILL NOT BE CLEARED UNTIL THESE ARE COMPLETE!
